Re: Ms Tabitha Taborlin, aged 45
I am writing to refere Ms Tobitha Tabarlin, who was diagnosed with type 1 diabetes mellitus, requires Continuing management at your facility after being discharged today.
Ms. Taborlin was admitted on 2 April with the episode of diabetic ketoacidasis which was treated With IV fluid and insulin medicines. His condition was stable and she was discharged from hospital
In term of her medical history, Ms Taborlin has long history of tybe 1 diabetes mellitus and hypertension" for which she was commenced on 45 units Pantus nightly and humalog with 12 units nutritional baseline. She was non Compliance with medication and diet. She is underweight 48 kg and has a habit of smoking.
On her today's visit. Mr Tabarlin condition was found to be stable but it is noticed that she was not following the recommended diet Education of the diabetes necessity of glucose testing, insulin administration and
Smoking cessation have provided. Along with that microvascular complication of diabeted has, discussed. Educational pamphlets and refills for Lantus and humalog has been provided. Please note, Ms Tabarilin is schduled to follow. -up in one month.
