Re: Ms Joan Cummings, aged 81
I am referring Ms Cummings who requires ongoing management for her multiple health issues at your facility following her discharge today.
Ms Cummings is a widow and lives with daughter who is 61 years old . Her family members are finding difficulty in providing care for her at home. She has had dementia since 2008 and she had mastectomy for breast cancer on 2000. Recently, she had fracture at left neck of femur due to fall.
On 29 July 2011, Ms Cummings was admitted for above mentioned issues and had CT scan for head . During hospitalization,She was observed for fall and evaluated her regular medications. She was mobilized with walker and hip protector. In addition , she was partially assisted with daily living activities. Therefore, exercise program was arranged with physiotherapist. She also commenced on an analgesics , ariept , 10mg and coloxyl and senna at night along with caltrate Plus two tablets in the morning.
It would be greatly appreciated if you could continue physiotherapy to promote optimal level of independence with activities of daily living and mobility. Kindly, follow up appointments with physician and optometrist.
