Re : Ms Tabitha Taborlin ,aged 45


Ms Tabitha Taborlin has been diagnosed with type 1 diabetes mellitus since childhood , requires assessment and management from your facility.She is being discharged today.


Ms Taborlin was admitted six days before with definitive symptoms of diabetic ketoacidosis. On examination,her blood glucose level was 530 mail/L . During hospitalization,educated her with the advantages of glucose testing, insulin administration and quite smoking,how ever, she has poor compliance with insulin injection.


Inaddition , discussed regarding microvascular and macrovascular complications of diabetes.Her recovery is satisfactory,but, she is not following diabetes diet.


Ms Taborlin lives alone in single flat.She is underweight to her age,her body mass index is 18.2 and she has a habits of smoking two cigarette per day.


In view of the above,it could be beneficial to visit Ms Taborlin and provide education regarding pamphlets and refills of insulin injection.It is imperative to refer the endocrine specialist for glycemic control and possible changes of insulin pump. Please note that she has follow up appointment after one month.
