Re: Ms Patricia Cooper, aged 67


I am referring Ms Cooper who requires urgent assessment of swallowing function and nutritional status since she has high risk of aspiration.


Ms Cooper is a new resident at our facility and she has no friends, but emotionally dependent on nursing staff. She has multiple health issues including diabetes with irregular diet pattern. She gained 8 kgs of weight in 5 months , so her body mass index is 30 due to increased appetite. She has no dentures and has no allergies to medication or food.


On 14 March 2019, Ms Cooper had chest infection along with occasional cough and shortness of breath. Yesterday , she underwent sporadic throat clearing and found food items with milk content. Today, she had an episode of choking during breakfast and turned distressed, struggled with cough . However she became normal after removal of food item which caused chocking.


In view of the above mentioned information, kindly make a visit to evaluate her nutritional status and provide intervention to prevent aspiration.
