RE: Mr Jon Alex Frederick, DOB 18 June 1987


The request is being written regarding the above-mentioned patient, who has injured his left knee as a result, difficulty walking, and requires home visits to improve mobility.

The patient has a history of Stroke since 2016 :therefore he had impaired balance and walks with a limp. A week ago, he had left knee injury following a fall from the stairs, consequently, he was admitted and recovered well with the treatment. The wound has healed , no sign of infection is noticed. The patient has been advised to walk with a walking stick. Though, he has been observed walking a small distance with the help of wife but still assistance and guidance is required.


Pain is reported during the walk ;therefore; a concern has been raised from the wife and she requested a physiotherapist home visit in order to improve mobility.


It would be appreciated if you can visit the patient, For mobility improvement and speedy recovery, assistance is required.
