Dear Mr Simon,
Re: Mr Jamie Morgan, DOB: 22 February 2017
Height: 177 Centimeter, Weight: 96 Kilogram
I am referring Mr Morgan, who underwent the Coronary artery bypass graft, requires your expert education regarding low-fat dietary guidelines, and healthy food
preference following his discharge.
On 27 September 2017, Mr Morgan was admitted to hospital with symptoms persistive of a obstructive coronary artery disease. After the assessment, the diagnosis was confirmed and the surgery was performed on 29 September 2017. Postoperatively, excellent performance of the wound has been noticed. Also, refrain from smoking, reduction of alcohol consumption, and low-fat diet instruction were provided. During hospitalization, he was reviewed by a social worker, and the improvement of the quality of life was discussed.
Mr Morgan is a single man, who lives his own house, is smokes 30 cigars per day, and consumes alcohol excessively. In addition, he eats deep-fried foods, burgers, eggs, and Mac Donalds.
About the above-mentioned circumstances, kindly provide proper dietary instructions. He has been scheduled for a follow-up with a local GP on 10 October 2017 at 2 pm. Arrangement of a local physiotherapist to continue the rehabilitation exercise program would be profoundly beneficial.
If you require any additional information, please do not hesitate to contact me at any time.
Yours sincerely,
Registered Nurse
