Re: Mr Alfred Billy, aged 52
Age : 52 years
I am writing to refer Mr Billy who requires continued care and mangement. He has been diagnosed with Basal cell carcinoma and being discharged today.
Mr Billy was admitted on 21 March 2010. During hospitalization, skin biopsy were performed to confirm the diagnosis, For which he underwent an operation CEE removal (cure it eye electrodissection). He has been commenced in panadein Forte 500mg for pain. Currently he has made good progres and ready for discharge.
Mr Billy, a Truck driver lives with wife . Mr Billy has had habits of alcohol comsumption for 30 years and smoking.
It would be appreciated, if you could consider Mr Billy by providing daily observation. It is to be noted that his medications to continue for one more week.
