Dear Ms Bruin,
Re: Mr Gerald Baker,aged 79
Mr Baker, a known case of yours requires , ongoing care and further management as he is recovering from left total hip replacement surgery.
Since 2016, he has been diagnosed with Osteoarthritis and was later worsened resulting in mobilization difficulties as a result ended up with THR on 04 sep 2018. Post operatively Aspirin 100mg was recommenced, along with his ongoing hypertensive management with Ramipril 5 mg in the morning, and if there is any complaints of pain Panadeine Forte 2tab qid to be given if necessary.
Post operatively good mobility was noticed with the use of wheele- walker. Due to anesthetic effect, disorientation of time and place was seen post operatively, and also HB dropped to 72g/dl which later required transfusion of three units of packed red blood cells. So continuing monitoring for anaemia is recommended.
Daily dressing at the site of surgery is needed for maintaining good skin integrity and continuing exercise program like strengthening and stretching exercises is highly recommended along with occupational therapy.
Based on the above, follow up is required after two weeks for staples removal and also for blood tests . For mobilization requirements a wheelie-walker and a pillow is provided by the hospital and toilet raiser is hired for 2 weeks by the social worker.
