Dear Ms. Bruin,


Re: Mr Gerald Baker; aged 79


I am updating you regarding Mr Baker who was admitted on 03 Spetmeber, 2018 and underwent left hip replacement surgery. He requires your further care and management form your facility following his discharge today.


Post operatively, Mr Baker has been commenced on aspirin 100mg in the morning, ramipril, 5 mg in the morning and panadeine forte four times a day for hypertension and pain respectively. Apart from this, his haemoglobin level dropped to 72/dl after surgery, for which three units of packed red blood cells were transfused. After that his haemoglobin level was stable; however, anaemia needs to be monitored.


In the recovery period, he was commenced on range of motion, stretching and strengthening exercises along with occuptional therapy, therefore he was able to walk length of ward with the help of a walker without difficulty. His wound healed due to daily dressings.


Upon discharge, he is provided with a wheelie walker and a wedge pillow. A toilet raiser has been arranged from the local medical supplier for two weeks. Please note, his staples need to be removed in two weeks and follow up FBE and UEC tests at City Hospital Clinic.


It would be appreciated if you could moniter his medications and preserve the slain intergrity and continue his exercise program as well.
