Dear Ms. Bruin,


Re: Mr Gerald Baker; aged 79


Mr Baker, who underwent a left total hip replacement, requires continuing care and support. He is being discharged back home today.


He has had osteoarthritis which was worsened; therefore, an elective surgery was performed. In addition, he has had hupertension for which ramipril 5mg has been commenced.


Post-operatively, aspirin 100mg was recommenced along with Panadeine Forte tablets were prescribed for pain relief. Daily wound dressings have been provided on the surgery site to preserve skin integrity. A series of motion, stretching and strengthening exercises as well as occupational therapy have been initiated . Post-operative disorientation was noticed regarding time and place which probably due to an anaesthetic effect. His Hb level was dropped to 72 and subsequently, three units of packed red blood cells were transfused.


Now, he is able to ambulate with the aid of a wheelie walker without difficulties. A wheelie-walker and a wedge pillow have been provided where as a toilet raiser has been orgained for a fortnite, Staples will be removed on 21 September and a follow-up FBE and UEC tests has been scheduled.
