Re: Mr Sean William, aged 72
I am writing to refer Mr Sean William, who has been diagnosed with COPD, reuire your advice and counselling after being discharged today.
Mr William presented to the hospital with complaints of shortness of breath, therefore required proper restvto hold his breathing. On assessment, he reported that salbutamol is not effective. His examination revealed that inhaler is not use appropriately. He was stopped salbutamole and ipratropium.
Mr Willaim is a retured insurance agent and lives alone. He has the history of leg ulcer and osteoarthritis respectively.
It would be greatle appreciated if you could continue his wound dressing and provide education regarding use of inhaler due to his relucatance to use the inhaler. Please note , his breathing pattern was ineffective.
