Re: Mr Martin Jones, aged 69
Mr Martin Jones presented with a complaint of difficulty to swallowing food and medicine who requires, prescription of syringe driver along with medicines for pain relief following a home visit.
On his first visit, body weight was 78 kg and comfortable on bed. On subsequent days, body weight was gradually decreased and reduced food intake although tolerate with fluids. Following days he was sleepy and less intake of food. Difficulty to taking medicines along with fluids and pain were noticed. Syringe drive was suggested but her wife reluctant to perform this. Topical cream and liquid morphine was advised for pain. His wife and daughter was frustrated regarding this situation and agreed to provided further treatment at home.
At present his condition is worsening because of complete stoppage of food, fluids and medicine.
In view of the above, kindly request to arranging a home visit for providing syringe on drive and administration of medication. Your special concern is highly appreciated.
