Dear Artner,
Re: Mr Nick Daniel, aged 68
I am writing to refer Mr Daniel, who has been recuperating from the right hip replacement surgery, requires further assisstance and management at you facility.
Mr Daniel initially presented on 13 May with complaints of severe pain on the right hip. Pain was not controlled with panadol. He was walking with limp. On examination, decreased internal rotation and felxion of the right hip were revealed; therefore, he was diagnosed with osteoarthritis on right hip. Tablet indomethagen 50mg for three times a day was commenced.
On 16 July he was admitted with complaints of decreased mascular mass ad range movement in right hip for which was underwent hip replacement surgery on 19 July. Post-operatively, uneasiness in hip area was noticed. He has been commenced with physiotherapy and he was probe to have dislocation. He has made good progress and now, is ready to be discharged.
Based on the aforementioned circumstances, sducate to Mr Daniel regarding how to walk with a walker. Kindly instruct that how to do weight bearing exercise and use to stairs after recovery would b greatly beneficial.
