Dear Ms. Brain,


Re: Ms. Gerald Baker, aged 79.


Ms. Gerald Baker, a known case of yours for providing Continuing care at nursing home following his discharge. He underwent left total hip replacement following an osteoarthritis.


Post operatively, he commenced pamipril 5mg at morning for his ongoing hypertension, panadoin foole 550mg at fourth hourly or if needed for pain and aspirin 100mg, at morning. During recovery period, he presented with complaintsof disorientation and confusion because of his hb was 72g/dl for which and units packed red blood cells were transfused. He was started with exercise are stretching and strengthening for improving range of motion as a result he attained good mobility after operation. However he was treated with occupational therapy also. At present he walks with wheel walker without any difficulty and his Hb was normal 112/dL subsequently ongoing monitoring is needed for his anemia.


In view of the above kindly request to monitoring for medication, exercise programme and his skin intregity. Kindly arrange for equipment of toilet raiser. In additon social worker will provide wheeler walker and wedge pillow with in 2 weeks hire from medical supplies. Please note staples to be remove on 21 September and there is a follow up FBE and UEC tests at City Hospital Clinic. Your special concer is highly appreciated.
