Re:Mr Craig Hems worth ,aged 72

Mr. Craig Hems worth –a –known case of yours ,is being discharged today for providing continuing monitoring and management under your supervision. He was admitted to our facility and was diagnosed ad tonic clonic seizure followed by sudden fainting.

On admission ,his wife reported that ,he was started to jerking on limbs ,apnoea for 20 minutes,bluish color on face along with incontinence of urine .He was regained consciousness after two minutes.On assessment he was noticed drowsiness ,disorientation although consciousness .Apart from this, bleeding from tongue, bruise on the left arm and knee. He was no neck stiffness and focal neurological signs were identified.

Mr.Hems worth transferred to ICU and was proceeded by neuro consultation. He was commenced inj.manitol 20mg iv BD,T-Clopilet 75 g BD,Inj TT IM Stat,Inj.phenytoin 10 mg iv bd,inj ceftriaxone 1.5mg iv bid for 5 days Amlodipine 5mg OD.His EEG and CT report were showed that focal epileptic variants and subdural hematoma on the right occipital region. During hospitalization h was provided by complete rest and ophthalmology consultation for his eyes. At present he is anxious about seizure while at home.

In view of the above kindly requested to provide continuous monitoring and management on his condition and medication. Also to advise him and his wife about use of safety alert band, regular sleep and healthy diet. Instruct him to avoid drive and strenuous activity.Please note that there is an appointment review for anticonvulsant therapy for two weeks.
