Iam writing to introduce Jake Peterson who needs to be monitored , he has concerns about a change to university soon and independent living . He has anxiety related with alopecia
and suspected of depression.
◦ Initially, Jake presented with hair loss in March, 2014 at the age of 13. Therefore,he was examined by a trichologist and commenced on steroids cream but, the medication was discontinued owing to his aggressive type of behavior and loss of sleep. His condition stabilized for four years and regrowth of hair started in irregular pattern.But on previous year onwards, restarted the condition and 6 kilogram of weight had reduced for the last 2 months.Although, he has shown less interest to consult with doctor for any treatments , started homeopathy and the result was negative.Subsequently,he has shown less interest in peer groping, not participated in sports activities and refused to attend the meeting with school counselor.
◦ Presently, his alcohol consumption has increased to 28 units per week and noticed for extreme anxiety.
◦ In line with the above matter,it is important to specify that close monitoring is essential and some special arrangements to be conducted related with his exam.
