Dear Registrar

Re: Amina Erica, aged 8

I am referring Amina Erica, who has been treated for meningococcal meningitis requires urgent admission at your facility for further investigation and management.

During Today's visit, lethorgy and restlessness were noticed. Vomiting and worsened headache were reported. Tachycardia and hyperpyrexia were noted. Urinary functions report was unremarkable. Although, full blood count revealed leukocylsis ( 18000 ). Additionally, neck stiffness and macula- popular rashes over legs observed. Hence, stat dose of penicillin injection was administered.

Initially,on 9 October 2010, Erica was brought with the complaints of pyrexia, rhinitis, cough and anorexia. All vital signs and physical examinations were normal. Viral infection was suspected and paracetamol e times daily is administered. On the subsequent visit, health condition became declined. Brufen 200mg was commenced and blood tests were ordered.

Amina Erica was a refugee from Somalia since 2005. Parents with 2 siblings are the family members. She has has a years ago and allergies were not repeated.

Based on the above mentioned circumstances,an urgent assessment and treatment for the improvement of Amina Erica's condition would be extremely beneficial. In case you require any additional information, please do not hesitate to contact me.

Yours faithfully
