Re: Mr Peter Dunbar, DOB: 18 march 1932

Mr Dunbar, has been diagnosed with 2 diabetes since 2016. Education was provided regarding the importance of diatery management like low fat low salt, sugar, calorie restriction, reduce alcohol intake and increase physical exercises. Along with metformin 500 mg three times daily, for hypertension Ramipril 5 mg daily were recommended. However, is reluctant to follow the recommendations and became non compliant to medications as well as to low glycemic diet. Poor exercise tolerance and restricted mobility were observed due to diabetic neuropathy 4 months ago.

Since the death of his wife last year, appears unmotivated, excessive consumption of alcohol, fat, salt and sugars were noted, blaming poor memory as the primary reason for his irregular intake of medication. Therefore suggestions were given to improve his memory. Daughter requires education recording diet, medications and exercises.

In June 2018, Mr Dunbar was admitted to the city Hospital in Newtown, ECG reveals myocardial infarction. As a result Post thromobolytic therapy atril fibrillation were observed, it was effectively managed with sotalol and warfarin, in addition hypertension was controlled by ramipiril 5 mg daily, will be moving to Centreville along with the daughter family.
