Re: Mr Peter Dunbar,DOB:18 March 1932
Mr Dunbar, requires unceasing monitoring of type 2 diabeties and encouragement regarding compliance with medication and dietory regimens. Following his transfer to your locality today.
On October 2018, Mr Dunbar presented with diabetic nephropathy, exercise intalerence and restricted mobility. Non compliant with medication regimen was noted. Blameing poor memory as the fundamental reason for negligence.
Since the death of his wife last year, excessive amounts of alcohol, fatty foods, salts and sujar have been consumed, contrary to the professional advice.
On September 2017, type 2 diabeties and diagnosed which was managed with tablet Metformin 500 mg. Alongside, hypertension was controlled with tablet ramipril 5 mg daily. Despite, giving education on compliance on diet, exercise and alcohol consumption 70h adenence for the same was noted.
On June 2018, Mr Dunbar was admitted to the city hospital with the complaint of a myocardial infraction. On the same admission,arterial fibrillation was diagnosed. Elecro cardingram confirmed the diagnosis. Consequently, commenced on tablet satalol 40 mg and tablet unefarin daily.
Owing to Dunbar's present condition, it would be extremely beneficial if you could provide regular monitoring state of affairs and encouragement on his diabetic diet.
