Re: Mrs Delma Eggleston,DOB: 18 March 1972


Mrs Eggleston,who has been diagnosed with stage 2 lung cancer, requires urgent treatment and further management is being referred into your facility today.


Mrs Eggleston, presented to the clinic with the complaints of shortness of breath,chest shoulder and back pain. Physical examination revealed,during each breath hoarseness of voice, productive cough associated with large amount of colour change sputum were observed. Additionally, persistent cough with intense chest pain reported. Chest x-ray and CT scan were requested reports showed cancer cells spread across lymph nodes with the tumer size 5.5cm.


Mrs Eggleston,lives with husband, interested in gardening and not a smoker and alcoholic. Occasionally has had betel quid not known allergy.


In terms of past medical history,type 2 diabeties mellitus, hypertension,and copD in 2002,2007 and 2012 respectively. On regular medication of Metformin 500mg three times a day and ramipril 5mg daily for hypertension.


Based on the above mentioned circumstances, your ongoing management and treatment would be extremely beneficial for Mrs Eggleston's present condition. In you require any additional information, please do not hesitate to contact me.
