I am writing to refer back mr gerald baker, who under went left total hip replacement . He requires continued care in your esteem facility . he is being discharged today.
mr baker was admitted to hospital with the complaints of difficulty in walking and climbing stairs. surgery done post operative he was disoriented possibly due to anaesthetic effect . he was transfused with 3 units of packed red blood cells as his hemoglobin dropped to 72 g ldl.post operatively he was commenceted as aspirin 100mg , panadaine forte 550mg along with ramipril 50mg fav his hypertension management. dressing applied daily an incision site. range of motion, stretening and strengtheing exercises , as well as occupationl therapy was initiated . his mobility noticed to be improve . he is abled to walk length of ward without difficulty with wheelie walker. he is fits to return back to nursing home . with wheelie walker and wedge pillow . toilet raiser was arranged for 2 weeks by social worker from local supplier.
it would be greatly appreciated , if you could monitor his pain medication . please note, he has follow up visit for his full blood count and urea electrdyte test at city hospital clinic . in addition , he is scheduled for staples removal on.
