Dear Nurse
Re: Mr Peter Dunbar , who has been diagnosed with type 2 diabetes mellitus, requires ongoing regular monitoring of medication compliance and encouragement regarding dietary regimen, is being transferred in to your care . Mr Dunbar is going to live with his daughter in centreville.
Mr Dunbar has a long history of diabetes mellitus which recently developed into signs of diabetes neuropathy and subsequently managing with metformin 500mg daily. Amoxicillin is prescribed for respiratory infection which was recently noticed continues adherence to medication regimen is replaced noticed continuous adherence to medication regimen is reported. Also have regular problems with dietary control over Salt, sugar, alcohol and fatty foods.
Since the death of his wife, he has been poorly managing diet and consumption of alcohol doses and takes intermittently , poor memory is concerned as a reason of neglect. In June 2018, mr Dunbar got hospitalised and myocardial infraction was confirmed by ECG and consequently taking treatment: satalol and Warfarin are commenced
In addition, ramipril 5 mg daily hypertension. Daughter requires guidance regarding dietary education and monitoring.
Based on above mentioned circumstances, appreciate monitoring and management regarding diabetes and dietary regimen for Mr Dunbar's health condition would be extremely beneficial.
