Re: Mrs. Brulcie Wood, aged 66
I am referring Mrs. Wood signs and symptoms are suggestive of heart flutter, for further evaluation and management.
Regarding her medical history one month ago Mrs. Wood had a uncomfortable feeling in her chest several times in the last few weeks like a heart flutter, on obeservation her blood pressure was 145/75, pulse rate 80, electrocardiogram and respiratory examination revaluation normal . She is an zocor 20mg daily including calciumcaltrate in addition prescribed noten 50mg 1/2 table to daily, adviced to monitorand record of frequent fibrillation sensation , follow up after two weeks. However , she doesnot smoke , occationally drinks alcohol. Regardingher family history, mother died with myocardial infection.
On 17 July, Mrs. Wood presented with less sensation, last weeks she had an attack, two times during night. On examination blood pressure was 135/7t, pulse ratio 70, advice to increase noted to 50 mg 1/2 tablet two tines every day and review after one month.
Today Mrs. Wood presented with a complaints of flutter attach frequently since last 3 days, physical examination reveal increased blood pressure 180/90pulse ratio.
Based on the above mentioned clinical features , your further management is highly appreciated.
