Re: Mr. Gerald Baker, aged 79
This is to update the current condition of Mr. Gerald Baker, who underwent left total hip replacement. He requires confined care in your esteem facility is being discharged today.
On 03 September, Mr. Baker was admitted to hospital due to worsening osteoarthritis for elective surgery. He was noticed to be disoriented, possibly caused by anaesthetic effect. He was transfused with three works of packed red blood cells as his haemoglobin dropped to 72g/dl. Post operatively he was commenced an aspirin 100mg and panadeine farte for pain management along with his antihypertensive medicine. Applied dressing daily on incision site. Range of motion, stretching and strengthening exercise as well as occupational therapy was initiated. His mobility progress ks remarkably improving. He fits to return to recovering home with wheelie walker and wedge pillow. Toilet raisers were arranged for sweets by social workers from a local supplier.
Based on above mentioned circumstances , monitoring of his medication complaince and monitoring skin inegritity would be extremely beneficial. Kindly note , he has follow up visit for full blood count and electrolytes test of city hospital clinic and scheduled for staples removal.
