Re: Ms. Sheila Dyer, aged 72
I am referring Ms. Sheila Dyer, who has been diagnosed with left - side Ischemic stroke and Middle Cerebral Artery, requires appropriate ongoing nursing care and management, being discharged back to your facility today.
On 10 March, Ms. Dyer was admitted to the hospital with high blood pressure 165/85 units. Tissue plasminogen activator was given at the samd time along with supplementary oxygen therapy, commenced on Aspirin 75 mg Enalapril 25mg once daily, dipyramidal 200 mg, Metoprolol 50 mg,Dabigatran 75mg twice daily, Atorvastatin 40mg twice daily at night to be continues till LDL less than 70mg/dl. However upper extremely weakness, especially muscles weeknesss in right arm noticed. Therefore aphasia and apraxia condition still remained , not able to performed activities of daily living such as dressing, showering and toileting. Regular physiotherapy, followed by stretching and strengthening execises along with speech therapy were initiated.
Ms. Dyer, had had history of hypertension since 2011, and dyslipidemia 2015.
Based on above mentioned circumstances ; regular montoring of medication compliance, blood sugar , and blood pressure to keep less than 140/90 mm Hg is profoundly beneficial during her current health condition. Continues monitoring of thr weight and exercises program along with low cholesterol diet recommended. Kindly emboled her in some social activities.
