Dear Mr Webb,
Re: mrs Sheila Dyer, aged 72
I am writing regarding Mrs Sheila Dyer , who has been diagnosed with a left side ischemic stroke with middle cerebral artery . He requires ongoing care in your facility is being discharged today.
On 10 March 2019 mrs Dyer was admitted with the complaint of high blood pressure 165/105 mm 1 hg therefore initiated issue plasminogen activator and supplementary oxygen therapy .she was commenced on atarvastalin 40 mg twice daily at night to be continued 1 dl less the <70 mg / dl. On subsequent day , observed with the muscle weakness and difficultly in daily living activities therefore , recommend with regular physiotherapy stretching and strengthening exercises.
Mrs Dyer has a history of hypertension and dyslipidemia for which , she is on medication enlapril 25 mg metoprolal 50mg and dabigatron 75 mg twice a day . In addition , laaungag therapy was initiated for aphasia and aproxia.
Based on the above circumstances, places monitor medications compliance, blood sugar and blood pressure level. Please note that , she was scheduled for neuralogy appointment on 22 May. She requires dictition advice regarding weight loss and chalesteral level encouragement to participate in social activities.
