I am writing this letter to you ms jane simms, who has diagnosed progressive multiple sclerasis admitted on 26 july 2008 at south eastern rehabilitation hospital . she requires monitoring medicine and weight , maintain stain integrity and continue exercise . she is being discharge today.

on admission she has had been experinced multiple sclerosis and rapidly increased weight suffered with depression and had pressure sore on high buttock .

Resume previously medicine and commenced carticosteraids and prothiaden 150 mg a day. preformed daily dressing on pressure ulcer area. Low calorie diet and exercise recommened observed range of motion stretching and stregthening . provided occupational therapy . she had improved and now discharge to home.

Based on above all information, she requires continue monitoring of medicine , maintain skin integrity and weight assistance for daily living activities , encourage to particepate social activites.
