Re: Mrs. Anne Ellis, aged 76


Dear Mr. Ferris


I am referring Mrs. Anne Ellis, recovering from bilateral multiple renal calculi and moderate hydronephrosis. Who requires your continuing care and management from your facility as she is set to be discharged today.


Initially, Mrs. Ellis administered to hospital on 12 June 2018 with complaints of intense pain on left loin and groin me radiating to lower back. Therefore, computed tomography, urine analysis were performed which revealed aformentioned diagnosis. Mrs. Ellis had severe vomiting, restless, and sweating, along with communication vital signs were recovered. In that, blood pressure elevated from normal range. On assessment , Mrs.Ellis had severe dehydration due to vomiting for which, intreavenous fluid started for 48 hours . Anti analgesic and anti emetics were administered. On 13 June Mrs.Ellis was commenced on Tamsolusin 8mg twice daily, Paracetamol 500mg daily, 4-6 hours for ongoing symptoms and pain relief.


Consequently, level of pain reduced, blood pressure levels became normal. subsequently , Mr.Ellis planned to discharge today. To be continue Tamsolusin and paracetamol as prescribed of note, outpatient appointment due to an 20 July.


I would very much appreciate your further assessment and monitor medication complaince and urine output. Dietary modification is appropriate to maintain his salt intake and oxalate rich foods as well as to reduce animal protein intake.
