Re: Mr Edwin Bruker, aged 56
Mr Edwin Bruker, who been exhibiting suggestive symptoms of post stroke depression , requires evaluation and appropriate managment from your services.
Mr Bruker was admitted on 14 february 2021, with the complaints of facial droop , hemiplegia and slurred speech. During hospitalization , he was treated with thrombolytic therapy. since he exhibited moderate dysphara , he was refered to speech therapist . In addition, he has been commenced on asprin . currently, he is tolerating semi- solid diet and he needs minimal assistance for daily living activites.
On 20 February 2021, mr bruker was transferred to asborne paek rehabilitation center for physiotherapy and speech therapy . in addition , he was referred to direction as well as provided with assistive device. It is worth mention that his relatives has visited every alternative day.
On 01 may 2021 , mr bruker was found to be in depressed state with associative symptoms of poor apetite , poor grooming , limited social interaction, less interest in physiotherapy as well as medication intake. also , approximately 6 kilogram weight loss was noted . Even though he was provided with cegnitive behavioural therapy and counselling . He showed paranoid behaviour with defusion and lack of sleep. consequently, he attempted sucide.
Based on the above mentioned circumstances, your willingness to render an urgent evaluation and management . Mr bruker psychological needs would be highly beneficial . Relatives were acknowledge regarding his current status and they are agreed for psychratrist consultation.
