Re: Mrs Sydney Loten
DOB: 14 August 1946
Mrs Loten, who has been recovering from a colon polypectomy, is being referred into your facility. She requires your ongoing care and management following her discharge today.
On 11 April, Mrs Loten was admitted for the routine colonoscopy which revealed the presence of multiple polyps and the surgical procedure was performed. Postoperatively, despite excellent overall progress, respiratory distress and metabolic acidosis were noticed. Subsequently, she was shifted to the intensive care unit and then to the ward. Oxygen support at 3-5 litres and wound care have been commenced. In addition, anelgesics and laxtives have been administered. Amputation was possible with a cane and a walker. Continuation of the oxygen therapy of 2-6L through a nasal canula from the home and the use of incontinence pad have been suggested.
Being a widow, Mrs Loten lives with her son. She has had COPD, diabetes, hypertension for which, Ipratropium bromide inhaler, 1 puff; formoterol, 2 puffs, twice daily; Prednisone 40mg, Metformin, 500mg, twice daily; glipizide, 5mg, once daily and losartan 40mg, once daily are taken. Consumption of 1-2 bottles of beer every week and smoking of 5 cigarettes daily have been mentioned.
Based on the aforementioned circumstances, guidance and support of Mrs Loten's condition would be profoundly beneficial. Monitoring regarding not only the medication compliance but also oxygen therapy intake has been recommended. Daily dressings have been advised.
