Mr Gerald Baker is a 79-year-old patient on the ward of a hospital in which you are Charge Nurse.

Mr Baker was hospitalised on worsening of osteoarthritis ,therfore the surgery was performed. Post-operatively his haemoglobin levels were low ,therefore ,3 units of packed blood cells were transfuses. Besides that ,as his Blood pressure was elevated ,he wa smanaged with tablet ramipril,5mg and tablet aspirin,100mg.In addition to this tablet Panadein forte was administered to relieve his pain. He is being discharged today. As you are aware, Mr Baker was admitted to hospital on 03 September, 2018 with the complaints of difficulty in mobilization and climbing stairs.He underwent left total hip replacement surgery due to worsening of osteoarthritis.Post-operatively, he was disoriented with time and place.Because ,possibility of anesthetic effect.His hemoglobin was decreased and three units of red blood cells were transfused.consequently,she requires to monitor anaemia. Upon discharge, it would be greatly appreciated if you could monitor his medications specially the panadein forte which will be given four times a day if he complains of pain. Additionally, please observe his skin integrity by changing the wound dressing daily. Furthermore, he needs to continue his stretching and strengthening exercises program. The hospital will provide him with a wheelie- walker and a wedge pillow, in addition, the hospital’s social worker has organized a two- weeks hire of the toilet raiser. It is important to note that he has an appointment to see an occupational therapist in two weeks time for removal of staples. Finally, please follow up on his blood tests at City hospital clinic. He requires your continued care and support and he will be discharged today. Mr Baker was admitted to City hospital with a complaint of worsening osteoarthritis that caused him some functional disabilities. Therefore, the aforementioned procedure was performed. Postoperatively, he experienced confusion, that might be related to the anesthesia that was given to him, and his Haemoglobin level dropped to 72 g/dl for which 3 units of packed red blood cell were transfused.
