Iam writing to request daily home visit for Alicia Nesbitt, 6 years old, who was diagnosed with low birth weight and opioid dependence following discharge today
Alicia’s mother, Gemma has history of heroin dependence from the pasr 2 years. She is single mother, seperated from husband due to domestic violence and supported by mother. Previously, worked as sex-worker.
Alicia weighs 2.0 kg at birth, which elevated to 2.3 kg within 6 days. Heroin withdrawl completed by mother and baby without any complication. Bottle feeding is erraticated and reduction in appetite is noted therefore, regular visit are recommended until the baby’s weight and feeding’s have stabilised. Education on infant care is advised, as mother is prompted to care and poor parentral bonding is observed. During hospitalization, Gemma’s condition was mananged with the help of drug and alcohol teamhence, can be contacted in order to get integrated support to reduce risk of heroin use.
On the basis of above-mentioned circumstances, it would be greatly appreciated, if you could continuously monitor mother’s coping and psychological state. updating social worker during risk and providing safe environment to baby is suggested. Your prompt care would be beneficial.
