Dear Sir/ Madam
Re: Ms Riya Roy, DOB: 12 March 2018
Ms Riya Roy, who underwent a normal vaginal delivery is being referred for short term emergency accommodation and supportive services following the discharge today.
Ms Roy is single and a victim of depression since the death of her parents in an accident. She is no more in contact with her current boyfriend who is not the father of child. No close contact with family and friends has been reported. So, a referral to the clinical psychologist is required for psychological well-being and counselling regarding parents and boyfriend relationships dynamics.
Ms Roy has delivered a healthy female child with adequate weight gain. Currently, bottle food is preferred as the mother is hesitant for breastfeeding. Post-partum complications were unremarkable. Ms Roy is being advised to apply for single parent benefits for aid brochures have been provided.
Therefore, excellent postpartum progress is reported with minimal blood loss and recent haemoglobin is investigated as 13.5 g/l. For follow-up care liaison with local maternal health service is highly imperative for postnatal monitoring. Ms Roy neither smokes nor consumes alcohol.
Based on the aforementioned circumstances, your supportive services and appropriate ongoing management throughout the postnatal period of Ms Roy’s health condition would be profoundly beneficial. In case any additional information is required, please do not hesitate to contact me.
Yours faithfully
