Re: Ms Margaret Helen Martin, aged 81
Ms Martin , who has undergone a successful angioplasty today for her coronary artery disease and angina. She will be going back home tomorrow and requires the assistance from a community health nurse for ongoing home care.
In regards to her social background, Ms Martin lives in her own house and he is financially independent sedentary as well as inactive lifestyle has resulted in obesity and elevation in body mass index upto 29.5 was noted. Due to lack of association as well as interaction with family had lead her to loneliness. Currently, takes aspirin and Plavin for coronary artery diseases; betaloc and tritace to treat hypertension hence, continuation of medication is recommended
During hospitalization, concern regarding adjustment to homely environment would be possible is raised by ms martin. Post operatively, recovery after surgery was well-recuperated with no sign and symptoms of infection or bruising at catheter site is observed and periodic investigation is adviced following discharge.
As per the discharge plan, it would be greatly appreciated if you could provide low –calorie, high protein, low cholesterol and gluten – free diet in frequent small meals as preferred by the dietitian. Abundance fluid consumption is suggested. According to physiotherapist, regular light exercise for instance, short duration walk along with refraining from strenuous labour involving physical strength is esteemed.
