Re: Ms Sheila Dyer, aged : 72


This is to update the current health status of Ms Sheila Dyer, who has been diagnosed with left-side ischemic stroke. She required follow-up care and is being discharged into your facility today.


On 10 March, Ms Dyer was admitted with the definitive symptoms of aforementioned diagnosis and immediately treated with tissue plasminogen activator and oxygen supplementation. In addition, after two days, she was commenced on Aspirin,Enalapril once per day and dipyridamole,Metoprolol, Dabigatran,Atorvastatin twice per day.


During hospitalization, she had difficulty with activities of daily living due to upper extremity weakness. Besides that aphasia and apraxis were noticed. Therefore, physiotherapy and language therapy were initiated and which needs to be continued. However, she has made excellent progress in her condition.


Upon discharge, regular monitoring of medication compliance and blood pressure to be maintain less than 140/90 mm is profoundly beneficial. Please make sure to screen her for diabetes and depression. Kindly encourage her to particiate in social activities. It would be grateful if you could arrange appointments with neurologist on 22 May 2019 and dietician to reduce her BMI.
