Dear Dr Taylor
This is to update the health status of Mr Thomas Sheffield,who has been recuperating after Permanent Pacemaker Placement secondary to complete heart block.He requires ongoing care and management and is being discharged back to your care today.
On 21 January 2020,Mr Sheffield was admitted after syncopal attack while playing golf.On examination, electrocardiogram and inducement of medicine revealed diagnosis.Consequently,his heart rate showed 30-40 beats per minute. Therefore,he underwent aforementioned procedure.Post-operatively,ibuprofen 800mg thrice per day and as needed prescribed for pain.Daily dressing was performed.He has been advised to use arm sling and avoid lifting less than 10 pounds if weight for three days and two weeks respectively. Currently, he is able to walk with out support.Despite, minimal pain,he has been advised to discharge on non-narcotic pain medication.
Mr Sheffield has had mayocardial infarction since 2015. Additionally,he has a history of hypertension and dyslipidemia.Therefore,he is on medication.
Up on discharge, kindly monitor Mr Sheffield pain level.He has been advised physiotherapy after three weeks.Education has been recommended regarding details of remote monitoring of pacemaker and discharge medication prescription.Please provide cardiology nurse before discharge and follow-up with cardiologist in 7-10 days to inspect the pacemaker site.
