Dear Ms Bruin,
Mr Gerald Baker is being discharged back to you for continuity of care, following a total left hip replacement.
Mr Baker was admitted with worsened Osteoarthritis and underwent a total left hip replacement. Post-operatively he was disoriented due to anesthetic effect. Additionally, his hemoglobin dropped, therefore three units packed blood cells were transfused.Currently, his hemoglobin is within normal range and requires monitoring for aneamia.During hospitalisation, daily dressing was provided and range of motion exercises were encouraged along with occupational therapy. Consequently, he could bear weight with the use of wheelie walker.
Your expert care is highly aporeciated if you could monitor his medication compliance ( aspirin, ramipril and panadine forte). His skin integrity to be preserved and exercise programms are to be continued.Please note, the hospital will be provided a wheelie walker and hospital social worker hired toilet raiser for his use at nursing home. His staples removal is scheduled on 21 September and needs a follow-up with full blood count examination and urea, creatinine, electrolytes test at the City Hospital Clinic.
