Dear Ms Ward,


Re: Mrs Charlotte Prince, aged 86


I am writing to request home visit for Mrs Charlotte Prince, who was diagnosed with mild concussion and infected right toe . She requires continuing care following her discharge today.


On 2 December, Mrs Prince was admitted on precautionary basis after her fall at home. Her test revealed unremarkable. Regular wound cleaning and dressing were advised twice a week. Her current medications includes ramipril,10mg for her hypertension, amoxicllin,500mg for her infected toe and ibuprofen for her scoliosis as per requirement. Monitoring of her medication compliance would be highly appreciated.


Mrs Prince is a retired postal worker and lives alone in a three bedroom apartment. She is a heavy smoker and occasional drinker. She ambulates with walker and requires possible assistance for showering and dressing.


Upon discharge, home visit was advised twice a week. Provide Mrs Prince with adequate rest. Educate on hand and food hygiene. Supervise her GP's visit.


Based on the view of above circumstances, Monitoring of her wound,medication would be greatly beneficial to improve Mrs Prince's condition.
