Dear Ms Thomas,

Ms Julia Smith, who underwent a caesarean section, is underweight and being discharge today. I am referring her for nutritional assessment and expert management regarding dietary concerns.

On 10 November 2018, an antenatal assessment conducted revealed second pregnancy. Ideal body weight of 32 kg was noticed, where as height was 164 cm and considerably underweight due to malnutrition. Inadequate intake of nutritional diet was reported regarding poor weight gain. Ms Smith is depressed of unplanned pregnancy at this early age, her education and health of baby.

In addition, due to consequences of poor nutrition, low birth weight of the baby was reported that is 1.9 kg. Also, a lot of complications have been noticed including risk for aspiration, bowel and bladder continence and baby is on nasogastric tube feeding. Current weight is confirmed as 37 kg on discharge.

Ms Smith is single and lives with her boy friend . Presently, no contact with parents have been reported. Due to life problems depression is also seen. Haemoglobin investigation which were performed showed mild anaemia (7mg/dl). Constant monitoring dietary compliance is highly imperative.

Based on the aforementioned circumstances, your expert assessment and appropriate management for nutritional balance for Ms Smith health condition would be prefoundly beneficial.
