Re: Ms Shella Catwright, aged 69
Ms Cartwright, who was diagnosed with unstable diabetes mellitus along with left foot ulcer. She requires unceasing care and life style modifications,following her discharge today.
Ms Cartwright, has been on oral hypoglycemics for the past nine years.Consequently, Glibenclamide five mg daily Metformin mg thrice daily were commenced. Augmentine Duo Forte hundred and twenty five mg orally twice daily was initiated.
Being retired, Ms Cartwright resides alone in her three- bedroom house. Since than alcohol consumption has increased and premium food intake has been reduced. Non compliance with self-administration of hypoglycemic medication was reported. In addition, her pension is very minimal. Subsequently, lasion her with community social service worker.
Ms Cartwright has nither friends nor relatives.Hence, involvement in social activities is imperative. Continuation of hypoglycemic agent is necessary. Review of antibiotic therapy on completion of current dose is recommended. Daily observation of ulcer, along with daily saline dressing is vital.
Upon discharge, adherence to medication and quality diet, along with monitoring of blood glucose and reduction in alcohol intake is required. Moderate exercise regime need to be encouraged. Simultaneously, income producing activity need to be instituted.
