Dear Ms Plasquale
Re: Mr George Chien, aged 54
Mr Chien, is being referred into your care and requires aid and support for his ongoing difficulties with mobility following a recent surgery.
On 10 January, Mr Chien was admitted with impaired self care activities, moderate swelling and fluid retention in the knee secondary to a right total knee replacement, which was performed on 27 December 2017, due to osteoarthritis.
In addition, complications related to postoperative anaemia were also noticed. Subsequently, ice pack and fondaparinux have been provided. Currently amputation is possible with a front wheel walker and haemoglobin level has been increased. However, minimal assistance in showering and body dressing have been perceived. Apart from this, anelgesics, thyroid support of 60 mcg and multiple supplement have been prescribed, which has to be continued.
Mr Chien lives in own resident and works as an accountant. Consumption of 2-3 units of alcohol has been reported.
Based on the aforementioned circumstances, comprehensive and rehabilitative care of Mr Chein's condition would be profoundly beneficial.
If you have any queries, please do not hesitate to contact me.
Yours sincerely
