Mr Schroder, who has been diagnosed with type 2 diabetes, requires ongoing care and support at home following his discharge today.
Today, Mr Schrodet was admitted with fatigue and anxiety symptoms, which have been noted for the past 6 months. He underwent tests such as HbA1c 72mmol/ mole and urine analysis report was negative for glucose and ketone. Appropriate counselling about blood sugar monitoring and hypoglycaemia were provided. Metformin 500 was advised daily for diabetes.
Mr Schroder lives alone and has no contact with family and friends. He consumes alcohol and smokes cigarettes quite often.
Up on discharge, he was advised to quit smoker and alcohol habits. Kindly provide him dietary management and support at home. In case case of any emergency, please contact Dr Keeling.
