I am writing to refer Mrs Mandy Cheng, who presented with chest pain, sweating, shortness of breath and pain in right shoulder. Your further assessment and management would be appreciated.


On 23 September 2020, Mrs Cheng was brought to emergency department with various complaint. During assessment, vitals signs and ECG were checked which revealed pulse 98 b/m, spo2 90% on room air and ST segment elevation. Acute myocardial infarction were suspected. After all the investigation, she was shifted to cath lab for PCI (Percutaneous coronary intervention) where two stents for blocked right coronary artery and kept for 24 hours observation and satisfactory. Her blood sugar was checked and reported 160mg/dl which was managed with Metformin. On 24 September 2020,she has been shifted to cardiac ward and advised to do fasting lipid profile tomorrow morning.


Mrs Cheng has had diabetes type 2 from two years for which Metformin 850 mg twice a day. The medication regimen aspirin 75mg, Atorvastation and Clopidogrel 150 mg once a day. In fasting lipid profile detect high-density lipoprotein.


Based on the aforementioned circumstances, your immediate observation and appropriate management of Ms Robert's health and condition would be profoundly beneficial. In case any additional information is required, please do not hesitate to contact me.
