Dear Mrs Hessal,
I am writing to refer Mr Schroeder who requires care and support regarding dietary management from your facility following his discharge.
On 13 September, 2018 Mr Schroeder presented with a six-month history of fatigue and anxiety type symptoms.On examination, urinalysis was done for glucose and ketones which was negative.His BMI was 32 kg/m2 and HbA1c of 72 mmol/mol were noted. He has been diagnosed with type 2 diabetes, for which he has been commenced on metformin, 500 my once a day.
Mr Schroder lives alone and has less contact with his friends and neighbours.He is a chronic alcoholic and a chain smoker. He has a remarkable family history of diabetes.
Upon discharge, Mr Schroder was given appropriate counselling regarding blood sugar monitoring and hypoglycemia as well as discharge medicines were explained. Apart from this, he has been advised to quit smoking and alocholism .
In view of the above circumstances, it would be greatly appreciated if you could provide education regarding dietary management and support him. Please note he has a review on 27 September, 2018 for general assessment and please contact Doctor Keeling in case of emergency.
