This letter is being written to refer Ms. Elma Edith, who was diagnosed with early stage cirrhosis of liver, requires an assistance to meet personal care requirements from your facility following discharge to home today.

On 17 June 2010, Ms. Edith was admitted with complaints of nausea, vomitinf associated with right upper quadrant abdominal pain. During hospitalization, symptomatic treatment was provided exception of surgical management, weight loss approximately 3 kilogram was noted. Currently, excellent health progress was observed.

Ms. Edith lives with her husband, who is disabled. In terms of her past medical history, she has had hypertension since 1994 but it has been controlled. Also, she was treated for jaundice in 2009.

Based on the aforementioned circumstances, an arrangement of ther nursing services for Ms. Edith to meet the personal care and monitoring of her condition would be extremely beneficial.
