Re: Mr Robyn Hardwood ,DOB :04 February 1915 .
I am writing to refer Mr Harwood ,who was diagnosed right partial rotator cuff tear ,as he requires follow up care and management from your service after being discharged today.
Mr Harwood was presented to the hospital on 30 June, 2011 with complaints of pain and weakness in the right shoulder following a fall from stairs at home. An x ray and magnetic resonance image confirmed above mentioned diagnosis . An orthopedic surgeons suggested for surgery but Mr Harwood referred for non-surgical treatment .He was commenced on tablet Ibuprofen four times daily and cortisone injections. Physiotherapy was performed daily .
Mr Harwood live alone .He is known case of diabetes mellitus type 2 for which he takes tablet metformin 500 mg morning.
In view of the above details, it would be greatly appreciated if you could monitor Mr Harwood blood glucose level every four hour which may be elevated because of cortisone injection. Kindly assist him in his activities of daily living. Please note that he has an appointment with Orthopaedic on 1 August ,0 2011 .Could you please visit his home address 8th peach st.
