Dear Sir/Madam,
Re: Mr. Robin Williams aged 80
I am writing to request a home visit for Mr. Robin Williams who requires an evaluation for his condition. His symptomes suggestive of Gatritis.
During my home visit today, Mr. Williams was having abdominal pain, vomiting and loss of apetite. He was seen by GP and was advised to take antacid and paracetamol 500mg TDS. Mr. Williams has complaints of vomiting mostly after eating. His vital parameters shows temperaturr is 38, BP and respiration is normal and SPO2 is 99% on room air. On examination it shows abdominal distention and poor peristatic movement.
Mr. Williams is a retired police officer. He is a widower and lives alone. He has a habit of smoking and smokes around 20 cigarates in a day.
He has a normal eating habit and have a preference of spicy foods. His BMI is 26.2 . He has had diabetes mellitus since 2008 and is on metformin. He had pepric ulcer on February 2020 and was treated successfully.
In view of the information presented above, it would be appreciated if you could provide a home visit for Mr. Willaims as he has fear of hospitalization.
