Mr Dawson, who has been recuperating from cerebrovascular accident, requires advice and assistance to improve his mobility. I am writing on behalf of his wife Mrs Elizabeth.
The couple live in their own home. Both are receiving aged pension. Mr Dawson is a world war 11veteran with gold health card entitlement. He had cerebrovascular accident 4 years ago. Currently he is mentally allert with slight speech impairment. His walking also slightly impaired. He is not using walker properly.
On 18 May 2008, Mr Dawson had a fall from the stairs and badly grazed on his left knee. Daily wound dressing and assistance for showering have been initiated. On the subsequent days, knee healing well and no signs of infection were noticed. With the help of his wife, Ms Dawson can able to get around the house. However, usual aches and pains are still persisting.
Based on the aforementioned circumstances, providing a home visit to assess his condition and assistance with improving his mobility would be greatly appreciated. His home address is enclosed for your reference.
