Re: Ms Mavis Brampton, aged 72


I am writing to refer Ms Mavis Brampton, who requires monitoring and ongoing care of cessation of smoking followed by her discharged today.


Ms Brampton is widow and socially active person. She is president of PROBUS in her area and attends the community centre three times a week. She has habit of smoking cigarettes ten per a day from the age of eighteen.


On 10 January 2011, Ms Brampton was admitted with complaints of fever and breathlessness. On examination , her blood pressure was 170/90 mm of hg and chest x-ray and sputum culture was done, Therefore she was supplemented with oxygen. During hospitalization, she was treated with intravenous Amoxicillin, Pravastatin 20mg per day and Celecoxib, 100mg per day. She also started on deep breathing exercises along with Nicotine patches and Zyban, 150mg twice in a day.


Upon discharge, kindly arrange for assistance in house keeping and shopping along with meals on wheels. Monitor her medication compliance and quit smoking plans which includes watch for side effects of Zyban, if it is stop immediately and if not continue for two months. Of note, Nicotine patches to be continue until smoking addiction is under control.
