Re: Ms Patricia Styles, DOB: 27 April 1957
I am writing to refer Ms Styles, who requires your urgent assessment and further management at your facility. She is being transferred today.
During today's home visit, Ms Styles informed chest pain which was relieved while sitting up and also complaints of shortness of breath and fatigue. She has low-grade fever (38.1 degree celcius), tachypnea (28 bpm) and tachycardia (115 bpm). Her blood pressure was 125/78, lower than her usual 140/90.
On 23 August 2019, he was unwell and was admitted to Green Valley Hospital with the complaints of fever, chest pain, fatigue and tachycardia. Throat swab was performed and detected viral influenza type B and echocardiogram shows pericarditis. Normal saline were commenced and Ibuprofen 600mg every eight hours.
Ms Style was diagnosed with hypertension, diabetes type 2 and depression, which was managed Accupril 40 mg twice daily, Diabex 500mg twice a day and APO-Gliclazide MR 30 mg orally daily.
In view of the above details, it would be greatly appreciated if you could do an immediate assessment and provide treatment of pericarditis.
